Once you have familiarized youself with the basic rules and regulations, you can download one of the Transfer Packages below which will provide all of the forms that you need to initiate your transfer application to the MOHA. Keep in mind that it will take some time to gather this information, so PLAN EARLY!
It is also important that the information you provide be accurate, that you follow the rules and that you submit your transfer application as one complete package. By doing this you will have the best chance of expediting the process.
